Who We Are

Home Heart Florida provides pediatric home health care for medically complex children and the families who care for them.

We know how personal it is to bring care into your home. Families are not just looking for a nurse or an agency. They are looking for people they can trust with their child, their home, and their daily life.

Our job is to make that feel less overwhelming. We help families understand their options, complete the required steps, coordinate care, and build a care team around their child’s needs.

Home Heart Florida was founded in 2023 by Arianna after she spent time caring for someone close to her with complex medical needs.

That experience changed the way she understood home care. She saw how much families are expected to manage on their own: finding nurses, coordinating schedules, keeping up with paperwork, communicating with doctors and insurance, and learning how to care for someone with complex needs at home.

Arianna saw the heart behind this work. She saw nurses and caregivers showing up every day. She saw parents becoming experts in their child’s care. And she saw families doing everything they could to keep their child safe, comfortable, and cared for at home.

But she also saw how hard the process can be when families do not have enough support around them.

Home Heart was created to help close that gap. We wanted to build a pediatric home health agency that felt more organized, more responsive, and more personal. A team that helped with the clinical care, but also with the many steps around it.

Today, that is still why we exist: to help children receive safe care at home and to help families feel supported through the process.

Home Heart Florida supports children and young adults with complex medical needs who require care at home.

We commonly work with families whose children need support with:

  • Family caregiver training (FHHA)

  • Private duty nursing

  • Care after a hospital discharge

  • G-tube, trach, vent, seizure, respiratory, feeding, or medication support

  • Help coordinating with doctors, case managers, and insurance plans

  • Guidance through Medicaid, managed care, authorizations, and required documentation

Families do not need to know exactly what services they qualify for before reaching out. Our team can help review your child’s needs, explain the options, and walk you through the next steps.

Home health can come with a lot of forms, approvals, phone calls, and next steps. We help families understand what needs to happen, what we are waiting on, and what comes next.

We build care around your child

Every child's needs are different. We take the time to understand your child's care needs, your home, your schedule, and your preferences before building a care team.

We support the people providing care

Great care depends on the people delivering it. We support our nurses and caregivers with training, communication, and guidance so they feel prepared and families feel confident.

We stay involved

Families should not feel like they are being passed from person to person. Our team stays close through intake, onboarding, staffing, and ongoing care so families know who to contact when they need help.

We care about the whole family

When a child has complex medical needs, it affects the entire household. We strive to make care at home feel safer, more organized, and less stressful for the family as a whole.
