Caregiver Resource Guide
A fillable guide for organizing your child's key care information in one place — patient and insurance details, a full list of doctors and specialists, a medication tracker, a weekly nutrition log, medical equipment provider contacts, and an emergency preparedness plan with a go-bag checklist and evacuation plan. Designed to be updated every 1–3 months or after appointments, and shared with family, nursing staff, and caregivers.
What to Pack in Your Hospital Bag
A packing checklist covering medical essentials (insurance card, medication list, medical records, doctor contact info, vaccination records, ID), clothing and comfort items (pajamas, a few changes of clothes, a favorite blanket or stuffed toy, toiletries), and fun and activities (books, coloring supplies, a tablet with downloaded shows, headphones, chargers).
